It’s on page 1-6 in Annex 11 (15th ed.) and in item 61 of article 2 of EU regulation 923/2012, as amended (the SERA regulation).

So, what’s the bottom line here?
CTR Should go to the surface. Those that don’t aren’t in compliance with ICAO.
The only effect that it has is that, for us, we can probably get a SVFR clearance in them, where otherwise we couldn’t.
