Yes, 5 years it is. Up to 2 years after expiry it is just as if it was still valid. Between 2 and 5 years it is subject to the AME’s assessment, and after 5 years you start from scratch, at least according to Part-MED.

Neither. Take a look at part-MED. This is a distant memory now, but from my recollection the state that issues the license must hold the medical records, ie. the form you fill out at the AME and any other documents you submit. So instead of your “foreign” AME sending your records to his supervising authority, he needs to send them to the authority that issued the pilot’s license.
